The dataset presented here was collected as part of the study "Developing and validating a vocational classroom-based assessment questionnaire for the vocational food and nutrition teachers." The purpose of the data set was to explore and develop as well as to validate the questionnaire. The study employed an exploratory sequential mixed methods design. The qualitative phase was exploratory, and the data collected in this phase was used for developing the questionnaire items. The quantitative phase was used for validation of the developed instrument. Qualitative data was collected using individual in-depth interviews with five food and nutrition teachers, cognitive interviews with seven teachers, and focus group interview (FGI) evaluations with six teachers in Botswana secondary schools who were purposively selected. The questionnaire items were developed and, with help from panel members, revised and pre-tested. The first pilot study was conducted on 30 teachers, and the results were analysed and adapted, and the final survey (second pilot) was administered to 101 teachers, and the results were evaluated using the Rasch Measurement Model (RMM).
